The Site provides general information about shoulder surgery services, pricing, and our practice. It is provided for informational purposes only and is not a substitute for professional medical advice, diagnosis, or treatment. Always seek the advice of a qualified physician with any questions you have about a medical condition.
Submitting the consultation request form does not create a doctor-patient relationship. A treatment relationship is established only after a paid consultation with one of our physicians.
Prices listed on the Site are estimates for cash-pay (self-pay) patients and represent bundled fees for facility, surgeon, and anesthesia at the time of publication. Listed prices do not apply when insurance is billed.
Actual prices may vary based on clinical findings during consultation, implants or devices required, and pre-operative imaging not included in the bundle. A formal written quote, valid for 60 days, is provided after consultation. Pricing excludes implants and pre-op imaging unless explicitly stated otherwise.
Outcomes vary by patient. Statements regarding success rates, recovery times, complication rates, and patient experiences describe general or historical results and are not promises or guarantees of any individual outcome.
Patient testimonials displayed on the Site are sourced from each surgeon's public profile and reflect individual patient experiences. Results vary based on patient condition, anatomy, and adherence to post-operative care. Patients quoted on the Site were not compensated for their statements. Testimonials are not a guarantee of similar results for any other patient.

These Terms are governed by the laws of the State of Ohio, without regard to its conflict-of-laws principles. Any dispute relating to the Site will be brought exclusively in the state or federal courts located in Mahoning County, Ohio, and you consent to that jurisdiction.
