Read this first: This site is a marketing inquiry funnel for self-pay shoulder surgery pricing. It is not a patient portal, electronic health record, or telemedicine platform. We deliberately do not ask about your medical condition, symptoms, diagnosis, or treatment history through this form. Diagnostic conversations occur only with a licensed clinician during your scheduled $250 virtual consultation, in a clinical setting subject to standard HIPAA protections.
1. What we collect on this site
When you submit the consultation request form, we collect:
- Identifying information: first name, last name, email address, phone number, postal code.
- Commercial intent signals: your timeline for scheduling (e.g., "within 1โ2 months") and your self-pay status (e.g., "paying out of pocket"). These are commercial questions about how and when you would like to engage with us, not medical questions.
- Service interest: which shoulder service category you indicated interest in pricing for. This is captured as a routing tag so our team can prepare appropriate pricing materials, but is not stored long-term as a queryable record field.
- Technical context: referring URL, UTM campaign tags, browser cookies for advertising attribution (Meta Pixel

2. How we use this information

- To improve the relevance of our advertising. Hashed identifying information may be transmitted to advertising platforms (e.g., Meta) via secure server-side conversion APIs solely for the purpose of measuring ad performance. We never transmit information about your service interest, condition, or medical context to advertising platforms.
- To maintain a record of your inquiry and our follow-up communications.
3. Where this information is stored
Information you submit through this form is processed by:
- Vercel โ the static website host and serverless function provider. Data passes through Vercel's infrastructure briefly during form submission but is not stored on Vercel.
- GoHighLevel ("GHL") โ our customer relationship management ("CRM") platform, where the contact record is created and where our team manages follow-up. GHL provides standard enterprise security including encryption in transit and at rest, access controls, and audit logging at the platform level.
- Meta โ only hashed identifiers (SHA-256 of email, phone, name) for ad-attribution purposes, never raw data and never service-interest data.
4. HIPAA scope of this site
The Orthopaedic Surgery Center is a HIPAA-covered entity, and the personal health information ("PHI") created during your clinical care โ including your consultation, diagnosis, and any procedures performed โ is protected under HIPAA in our clinical systems.
This marketing inquiry website is operated as a separate channel from our clinical systems. It is designed to collect minimal information at the inquiry stage. Once you become a patient and clinical information is created, that information is governed by our Notice of Privacy Practices, which you will be provided at the time of your consultation.
5. Cookies and advertising

- Contact you by phone, SMS, and email regarding the shoulder service category you indicated.

You may withdraw this authorization at any time by replying STOP to any SMS, clicking unsubscribe in any email, or calling 330.758.1065. Withdrawal does not affect actions already taken in reliance on your authorization.
7. Data retention
Marketing inquiry records โ including the data you submit through this form โ are retained in our CRM for the duration of our active follow-up plus a reasonable period thereafter. You may request deletion of your record at any time by contacting us using the information below.
8. Your rights
You have the right to:

- Withdraw your authorization at any time.
